Teacher Request Form

Let us pull materials for you to pick up at the library.

• This service is for teachers who are Champaign Public Library or Urbana Free Library cardholders as well as anyone who teaches in Champaign. Teachers who do not have a Champaign or Urbana library card may use their school’s library card to place a request.
• Submit up to 2 topics per request; request up to 20 items per topic.
• Items will be ready within 2 to 3 days; items will be held for 7 days.
  When items are ready for pickup, a notice will go to the phone or e-mail associated with the library card used to place the request.
